1. Yarn was inadvertently carried in front.
2. The loops are unwound back to the stitch in question which is held on a 3.0 mm crochet needle (closest size I had to 2-3/4 mm, the knitting needle used for the piece.) A dpn (shown in gold) was used to pull & hold yarn from the loops.
